Does anyone know how I can be alerted by email or anything else if the “App Health Check” changes from Available to Unavailable? for my digitalocean ap?

What you could do is to use the DigitalOcean uptime monitoring service:
Uptime checks your URL endpoints (any page on a website or an IP address) and alerts you when they’re acting slow and when they’re down.
Once you create an Uptime check, you will have the option to set up alerts for latency or downtime. On the alert creation page (see Create Checks) you can use the “Connect Slack” button to link your Slack account or email.
