So I have a R Shiny app running on an App via Docker at DO. It works great! However the app times-out after a while, which I do not want.
I have read that for example at a Droplet this imeout can be do undone because I have root access and make any configuration changes as needed.
How does this work with an App?

What is the time out value that you are interested in?
I believe that there is a hard set 100 seconds timeout limit in the App platform for requests that unfortunately can’t be changed.
If the App Platform’s constraints are too restrictive for your needs, you might consider deploying your R Shiny app on a Droplet. This will give you more control over the server environment, including the ability to configure timeout settings as you see fit.
As you already have a Dockerfile, you could create a Docker Droplet with 1-click and test it out:
